District nurses

District nurses are registered or enrolled nurses who are members of Family Health Teams providing a wide range of services in their local communities, often visiting people in their home.

Description

District nurses provide expert assessment, management, support and education for both ACC and non ACC referrals, for:

  • wound care
  • IV (intravenous) management
  • palliative/end of life care
  • ostomy care
  • continence assessment/catheter management
  • assessment for acute personal care and home help assessment (non-ACC)
  • support with home oxygen
  • health education
  • liaison with other health professionals.

Location

At community bases throughout the Waikato DHB district, and providing services in people’s homes and communities in all rural areas, towns and in Hamilton city.

Contact information

Hours of service: 8am – 4.30pm Monday to Sunday
Limited service available for patients identified by the district nurse as requiring contact after hours and on weekends.
Occasionally we may need to cancel a planned visit.  We will contact you and arrange a new time. (We will not cancel if it could compromise your care or recovery).

All referrals must come through the Community Regional Referral Centre fax: 07 858 1071
Self-referrals are not accepted. Referral is only through your GP or hospital service.
